ACC & Implant Replacement

Periodontal procedures are available to lay the groundwork for restorative and cosmetic dentistry, to improve function and the appearance of your patient’s smile.

ACC require complete documentation to support an applicant’s request for approval for implant replacement of teeth lost as the result of personal injury.

ACC is concerned about inappropriate patient selection and the number of patients who do not complete their treatment and now have uncrowned implants. They have indicated that nationwide 26% of implants were not restored last year (year ended June 2009 versus 17% the year before). As a consequence they now require that the patient’s informed consent letter includes a crown co-payment section. I write this letter as part of their initial consultation process – you will need to advise me of the co-payment amount in your referral letter, so I can add this detail to their letter.

You need to supply

  • Completed Section 2 of the ACC899 form (complete the other portion of the form)
  • Study models
  • OPG film- this needs to be less that 6 months old
  • A copy of your treatment records ideally dating back 5 years
  • If the patient has only seen you for a short period of time then they will need to obtain records from their previous dentist forwarding them to me
  • An indication of the patient’s co-payment that will be associated with the treatment proposed in Section 2 of the ACC 899 form. This should include their partial denture cost if one is going to be provided.

Please return this material to us (not to ACC) as we collate this with other records and forms to send to ACC.

If the patient is going to need a partial denture, it is worth either taking two sets of impressions or getting your technician to duplicate the models. The master set can be used to make the partial denture whilst the second set can be sent to ACC (via us).
